Insurance

I have in-network status with many, but not all, of the insurance plans offered by Premera Blue Cross, Regence Blue Shield, and other Blue Cross/Blue Shield plans. You should always verify my network status with the insurer.

I am out of network for all other insurers, and although I can assist with submission of claims, patients are individually responsible for timely payment.

I do not accept Medicare or Medicaid (including Apple Health). Persons covered by those plans can see me by paying privately, and are required to sign a contract agreeing not to submit those charges to Medicare or Medicaid. See the form “Office Policies.”

Initial visit	$350
Subsequent 45-53 min visit	$240-300
Subsequent 20-25 min visit	$200-250
Telephone management	$300/hr
Administrative and legal services	$400/hr

* Subject to change without notice Payment can be in the form of check, cash, or credit card. Past due accounts are subject to interest charges of 9% per year.
